• The Autism Care Navigator, Telephonic Behavioral Health, plays a crucial role in coordinating and facilitating care for beneficiaries with a primary diagnosis of Autism Spectrum Disorder (ASD) under the Autism Care Demonstration (ACD)
• As a primary advocate, the Care Manager will work with beneficiaries, families, and providers to ensure comprehensive and effective care
• Serve as a primary advocate for assigned beneficiaries, conducting assessments, developing comprehensive care plans, and coordinating medical, behavioral, and other services
• Collaborate with providers, caregivers, and other stakeholders to ensure treatment is initiated and barriers are addressed
• Utilize Motivational Interviewing and solution-oriented approaches in communication
• Participate in care management and coordination of services in an effective and efficient manner
• Develop comprehensive care plans in collaboration with identified stakeholders

Provide professional and courteous service to all callers and work to resolve any complaint or issue to their satisfaction

• U.S. citizenship, successfully receive interim approval for government security clearance, and obtain designation as a Certified Care Manager (CCM) within one year of hire
• Proficiency in Microsoft Office programs, specifically Word, Excel, and Outlook
• Skilled in written and verbal communications

Ability to handle high volume of calls and customer contacts in a polite and professional manner

• Active unrestricted Registered Nurse (RN) license without disciplinary action, with a minimum of three years of clinical experience in pediatrics, behavioral health, and/or ASD, or

Licensed Clinical Social Worker (LCSW), Licensed Masters Social Worker (LMSW-ACP), Clinical Psychologist, or Licensed Mental Health Counselor (LMHC) with a minimum of one year of post-degree clinical experience in pediatrics, behavioral health, and/or ASD
