- Shift: Day shift, 8-hour shifts
- The Home Health Registered Nurse provides skilled nursing care to patients in their homes within a post-acute care setting, ensuring high-quality, patient-centered care and accurate clinical documentation
- Deliver skilled nursing care in the home health setting
- Assess patient conditions and develop appropriate care plans
- Administer medications and treatments as ordered
- Educate patients and families on care plans and disease management
- Document all care accurately in the electronic medical record
- Every 3rd weekend and holiday rotation
One weekday after-hours call required
